About this resource
Which sight words a student knows, which they do not yet, and when you last checked. This is a
plain data tracking sheet, not a timed probe pack: there is no scoring strip, no aim line, and no
graph anywhere in this file.
WHAT IS INCLUDED - 17 pages, a word list template, five tracking sheet formats, a caseload roster,
a progress snapshot, a family update, and a phrase bank
Setup
- How This Tracking System Works - three steps and a table matching nine situations to the page that fits
- How the Type-In Fields Work - one page, so nobody emails you
- Word List Setup - a blank, numbered 20-word template; write in the words this student or group is
working on from whatever list or program your school uses (or your own list); every tracking sheet
in the file refers to a word by its number instead of reprinting the list
- Caseload Sight Word Roster - 14 rows: student, grade, word list in use, start date, next check due
Five Tracking Sheet Formats
- Individual Sight Word Tracking Sheet - 16 rows, word by word: date introduced, date last checked,
mastered, not yet, notes. The detailed, one-student version.
- Quick Check-In Log - 16 rows, one row per check instead of per word: date, words checked, count
mastered, count not yet, notes. The fast version for when there is no time for word-by-word detail.
- Small-Group Sight Word Tracking Sheet - 12 words by 5 students on one shared list, checked off as
each student masters each word.
- Caseload Mastery Overview - 14 rows, one per student: word list in use, words introduced, mastered,
not yet, date of last check. The whole caseload at a glance.
- Home Sight Word Practice Log - 10 rows for a family to fill in at home: date, words practiced,
knew it quickly, needed help, did not know it yet.
Reporting And Communication
- Mastery Progress Snapshot - 8 rows, review period by review period: words introduced, total
mastered, new this period, still not yet, next check date. No graph: counts only, for a meeting.
- Family and Team Update - a short note naming what a student is working on, what changed since the
last update, what to practice next, and when the next check is planned.
- Observation and Notes Phrase Bank - 18 observable-language sentence starters across five
categories (reading the word, when a word was missed, conditions and support, patterns across a
check, progress over several checks), every one ending in a blank for what was actually seen.
- A Tracking Sheet, Filled In - an invented second grader, five words on the list, the tracking sheet,
the progress snapshot, the quick check-in log, and the family update all built from the same five rows.
- Binder Cover and Divider Tabs - a cover page and four divider tabs (Setup, Tracking, Summary, Home)
to print, cut, and drop into a three-ring binder.
What This Is, And What It Is Not
It is a fillable PDF: open it in a PDF reader, click a line or a cell, type, and Save As once per
student or group. It works offline with no account or subscription. It is not a Google Sheets file,
a Google Doc, or an Excel workbook, and it does not count or calculate for you.
It is not the store's Functional Sight Words Progress Monitoring Probes pack: that file is six
timed probe forms with a scoring strip, a goal page, and a graph. This file has none of those; it
is the running record you keep between checks (or instead of a timed probe, for a student whose
plan does not call for one). It does not reproduce any publisher's or program's word list — every
word-list page is blank for you to write in the words this student is actually working on.
Who This Is For
Special education teachers, reading interventionists, case managers, and the paraprofessionals and
families who help a student practice sight words between checks.
