About this resource
The skill-growth tracker for a student working on conversation, friendship or peer-interaction goals. Not the check-in/check-out points program, not the ABC incident log — this one measures a conversation or friendship skill against a rubric, checks a set of specific friendship behaviors across the week, logs how a conflict was handled, and records a self- or peer-rating.
This is the clipboard for a school counselor, teacher, para or related-services provider tracking one peer-interaction or social-skills goal, as a fillable PDF. Type the roster, the rating scale, the strategy key, and the context codes once, Save As per student, and pen the checks and ratings during the day. Every line, box and cell is a type-in field; on a tablet the whole thing can be typed.
WHAT IS INCLUDED — 19 pages, fillable, 739 fields
SETUP (5 pages)
- Which Sheet, and What This File Is Not — match the goal's wording to a sheet once; the bottom row tells you which goal belongs elsewhere (a behavior-incident goal or a points program).
- How the Type-In Fields Work — ninety seconds of reading on what to type and what to pen.
- Social Skill Rating Scale — four levels the team defines for THIS goal, in observable behavior, not feeling.
- Peer Rating Scale, Conflict Strategy Key and Context Codes — a five-point self- or peer-report scale, six conflict resolution strategies from the plan, and six setting codes.
- Caseload Roster — every student, goal area, which log page, and the dates.
DATA SHEETS (5 pages)
- Conversation Skill Rubric and Observation Log — ten rows for observed conversations: the skill targeted, in your own words, rated against the scale on page 5.
- Friendship Skill Checklist — eight specific, observable friendship behaviors (inviting a peer to join, sharing without a reminder, offering a compliment, and five more), checked the day each was seen, plus a daily total row.
- Conflict Resolution Log — eight rows for what happened between students, the strategy tried, and how it ended; a blank line for your district's own reporting rule.
- Cooperative Play and Small-Group Observation Log — eight rows for role, skill rated against page 5, and prompt level during play or group work.
- Peer Interaction Rating Scale — ten rows for a self-report number, and a peer-report number only if your setting already collects one.
TRACKING AND REPORTING (4 pages)
- Cumulative Tracker — one goal, thirty-six observations across three blocks. The date, setting and rating from each log, copied so the trend is on one line.
- Progress Graph — hand-plotted, aim line first. A rating is plotted at ten times its value.
- Session / Observation Notes — two entries per page. What was targeted, data, how the student responded, what happens next.
- Quarterly Progress Summary — one student, reporting period; each goal with its baseline, current level from the tracker, a status code, and one sentence for the report.
HOME (1 page)
- Family Note — what was worked on in family words, something that went well with a peer, one thing to try at home, and a bottom half for family reply.
EXAMPLE (1 page)
- Eight observations and a friendship-checklist week for an invented fifth grader (D.K.), filled in. The example shows the status and cooperative-play observation request coming from cells already written, not from how the week felt.
What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a line, a box or a cell, type, press Tab to move on, and Save As once per student. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc, or an Excel workbook; it does not calculate or draw the graph. Prefer paper? Print it blank and pen the whole thing.
It is NOT a social skills curriculum, not a counseling protocol, not a tool for identifying a condition, and not a discipline procedure. No page names a condition or labels a student; every skill is written as an observable action, never as a clinical term. No rating level, checklist item or conflict resolution strategy is defined here — your team writes that in, from the plan and its own judgment. The sheets only make sure the number gets written down.
It is NOT this store's Behavior Data Collection Sheets Editable (ABC, frequency, interval and scatterplot for a target behavior — counting how often a behavior happens) and it is NOT the Check In Check Out Point Cards and Data Editable (a daily arrival/dismissal point program). Both track an incident or a point total; this file tracks a conversation or friendship skill growing over time. Both siblings are named on the chooser page and the back page so a buyer does not mistake one for another.
