About this resource

The beginning-of-year packet a family fills out and sends back, before anything else happens: a welcome letter, a two-part information sheet (household contacts and pickup names, then a plain-language page about your child), a phone or in-person interview version for a family who would rather talk it through, a quick reference card, a whole-room roster, a packet tracking log, starter phrases for a family unsure what to write, and a filled example. Set up once at the start of the year, and every other page in this file works from the same two sheets.

What This Is, And What It Is Not

It is a fillable PDF. Open it in a PDF reader, click a box or line, type, Tab to the next, and Save As with the student's name. It works offline with no account or subscription. It is NOT a Google Form, a Google Doc, or a district's online enrollment portal.

It never asks about eligibility, a label, or any condition a family has not chosen to name themselves, and it never arrives with any health information already typed in - the allergy and medical-notes line is blank in every copy, for the family to complete in their own words. It is not the Parent Communication Log - the dated, ongoing contact record for the rest of the school year once this packet is on file - and not the Parent Input Form, which is built around an annual review already on the calendar. This file is the one page, in several shapes, a family fills out before any of that.

Who This Is For

Classroom and special education teachers and case managers who want more than a name on a roster before the first day, front offices collecting the same household basics every August, and any team that wants a family's own words about their child on file in the first week instead of guessed at in October.
