About this resource

A student-led conference is a different job from a teacher-led one: the student presents their own work, their own goal, and what has changed, while the family listens and asks questions. This file is the student-facing paper for that — a coaching page for the teacher, a K–2 picture-and-circle form set and a grades 3+ write-it-out form set (each with a prep page, a goal page filled in with the teacher, and a talking-points page to read from), a phrase bank, a family invitation, a parent feedback slip, a one-page Conference Snapshot to send home, and a filled example.

What This Is, And What It Is Not

It is a fillable PDF. Open it in a PDF reader, click a line or a box, type, and print or save. It works offline with no account or subscription. It is NOT a Google Sheets file, Google Doc, or Excel workbook.

It is not a required format, a rule about which grade a student starts leading a conference, or a statement of who must attend one. A school's own conference schedule and any practices a team already uses go on the blank lines in these pages. Nothing scores, grades, or labels the student; a "still working on" is always one specific habit or skill. The example student is invented and labeled as such; no health data is pre-filled.

Who This Is For

General and special education teachers running student-led conferences at any grade band, and the families on the other side of the table.
