About this resource
Parent-teacher conference night usually means the teacher talks and the child sits quietly. In a student led conference, the child shows their own work, tells their own strengths and shares their own goal, while the family listens and asks questions. This file is the paper for every part of it, for general-education classrooms in grades 1-5: strengths and work-sample pages, a goal page for grades 1-2 and another for grades 3-5, a conference agenda the student checks off, a script for each grade band, a phrase bank of sentence starters, a family invitation, a family feedback page, a Conference Snapshot to send home, and a filled example.

What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a line or a box, type, and print or save. It works offline with no account or subscription. It is NOT a Google Sheets file, Google Doc, or Excel workbook.
It is not a required format and not a rule about what a conference must contain or who must attend. Your school's own schedule and practices go on the blank lines. Nothing scores, grades or labels a student. The example student is invented and labeled as such, and no real student information is pre-filled.
Who This Is For
General-education teachers in grades 1-5 who want the student to run the conference, and the families on the other side of the table.
