The clipboard a student holds while learning to watch their own behavior, work completion or executive-function goal, as one fillable PDF. The teacher types the goal in the student's words, the cue and the choices once and Saves As; the student writes the marks in pen. An on-task self-check with a teacher match column, daily self-rating sheets for six and eight periods, a work completion checklist, a morning routine and materials checklist, an executive function weekly checklist, a break and regulation log, a goal setting sheet, a weekly reflection, the teacher's match data sheet with the fade rule written in advance, a progress graph, a certificate, and a filled-in example.

Setup

What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a line, a box or a cell, type, press Tab to move on, and Save As once per student. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc or an Excel workbook, and it does not calculate percents or draw the graph.
It is not a behavior plan, a curriculum or a list of strategies. The teacher types the goal, the cue and the break choices; the student does the watching. Nothing in it says which students should self-monitor or why a behavior happens. If a plan or an IEP already names the goal, its words go on the setup page.
