About this resource
Ask the student first. This Student Transition Survey gives you a picture-supported version and a text version of the same four areas: school, work, living and community. The student points to a picture, says it, or ticks a box. Then a family page and a one-page team summary show what you heard. Type into it on your computer, or print and write by hand.
EVERYTHING INSIDE (21 pages)
The Survey, For The Student
- A picture survey: 4 pages, 8 picture cards a page, 32 cards in all. Each card has a picture, a big word and 3 tick boxes (I like it, Maybe, Not for me): 96 tick boxes
- A text survey: 2 pages, 12 sentence starters, 2 writing lines each
- A strengths page: 12 picture cards, with "I am good at this" and "I want to learn" boxes
- A big picks page: a top pick and a drawing box for each of the 4 areas
For The Family And The Team
- A family view: 8 questions with type-in boxes, and a "looking ahead" box
- A team summary: a tally table for the picture survey (I like it, Maybe, Not for me, top pick for each area) and a "what we heard" table
- A share-back note for the family and the team
- A phrase bank: 16 sentence starters in 4 groups, ending in blanks
- A filled example: a picture page and a team summary for an invented student, with every number checked against the cards
To Organize It
- Start Here, a how-to page, a binder cover with a spine strip, 8 divider tabs, terms of use, and a credits page
What This Is, And What It Is Not
It is a fillable PDF: click a line or a box and type, or print and write. It is NOT a slide deck or a document file. It is a survey, not a test, and it gives no score. The pictures are clip art from Whimsy Clips and Educlips. It carries no eligibility rules, timelines or ages: your district's rule is a blank line labeled "Your district's rule (write it here)." The student in the examples is invented, and the examples say so on the page.
Who This Is For
Special education teachers, case managers and aides who sit with a student to plan what comes after school, and the families who help. One classroom license.
