A token system has two halves: the plan the adults agree on, and the pages the student actually uses. This fillable PDF has both. Type the plan once, print a star board, cut out the tokens, and keep the log in the same file.

Plan The System

• Token Plan - six rows for target behaviors written in positive words, with where or when and tokens earned, plus token type, board size, earn rate, who hands out tokens, where the board is kept, exchange time, and three lines for the rules in the student's words
• Team Agreement and Fade Plan - five rows for the adults who give tokens, a place to write what every adult does when a token is not earned, five lines for the fade plan the team writes, and four team review rows
• Reward Menu Planner - twelve rows with reward, size, cost in tokens and where it is kept, plus lines for rewards the student picked and rewards not available

What This Is, And What It Is Not

It is a fillable PDF. Open it in a PDF reader, click a line or a box, type, and Save As once per student. It works offline with no account or subscription, and every page also prints for a pen.

It is not a behavior plan. It does not say how many tokens a student should earn, how rewards should be priced, what happens when a token is not earned, or when and how to fade the system. The team decides, and every one of those has a blank line. No health information is pre-filled anywhere, and the example student is invented.
