About this resource

A UFLI progress monitoring sheet for recording Checkpoint 1, 2, and 3 results for UFLI Foundations lessons, by skill area, with a plain visual mark you make by hand to show where each result fell. This is a blank score tracker, not a lesson plan: there is no UFLI lesson, passage, word list, or proprietary scope-and-sequence detail anywhere in this file, and it does not administer, score, or grade anything.

Five Tracking Sheet Formats

• Individual Student UFLI Checkpoint Tracker - one student, all six skill areas, a score column for each of the three checkpoints, plus a notes section for the year. The detailed, one-student version.
• Quick Checkpoint Score Entry Log - 22 rows, one row per check date instead of per skill area: date, skill area, checkpoint, score, mastery level, note. The fast version for when there is no time for the full tracker.
• Shared Checkpoint Monitoring Log - 16 rows, for the interventionist or general education teacher checking one skill area between checkpoints – a second adult's own copy of the running record.
• Small-Group UFLI Checkpoint Log - 14 rows, one shared skill area and checkpoint, 4 students tracked side by side as each check is given.
• Caseload Checkpoint Overview - 18 rows, one per student: current checkpoint, Overall score, mastery level, and last checkpoint date. The whole caseload at a glance.

What This Is, And What It Is Not

It is a fillable PDF: open it in a PDF reader, click a line or a cell, type, and Save As once per student or group. It works offline with no account or subscription. It is not a Google Sheets file, a Google Doc, or an Excel workbook, it does not connect to a UFLI data system, and it does not calculate, convert, or set any score, cut point, or mastery level for you.

It is NOT UFLI Foundations itself, and it does not administer, score, or reproduce any UFLI lesson, passage, word list, or proprietary scope-and-sequence detail. It is a standalone tracking document for use alongside UFLI Foundations. Every score field and every mastery mark is blank for you to fill in from a checkpoint result you already have in hand.

This resource is for use alongside UFLI Foundations to track a student's own checkpoint results. It is not affiliated with or endorsed by the University of Florida Literacy Institute. "UFLI" and "UFLI Foundations" are associated with the University of Florida Literacy Institute.
