About this resource

Need a behavior essay a student can actually work on? Hand over a page, and the student reads a short model essay about a behavior and copies it, or fills in a frame that asks three things: what happened, how it affected others, and what I will do next. Every student page opens with a "My name" line and one short instruction, and every writing box is ruled with real room to write. Print it, or type into it. There is a plan page, a words page, a self-check, and the teacher pages around it, so nothing has to be made from scratch.

EVERYTHING INSIDE (29 pages)

How A Student Uses It

1. The teacher finds the behavior on the topic list.

2. The student reads the model essay and copies it, or plans and fills in the frame.

3. The student checks the essay and talks it over with an adult.

4. The teacher logs it and, if wanted, sends the note home.

Three Shapes, Three Supports

A small circle, square or triangle by each page title shows how much help the page gives, so students are not comparing numbers. Circle pages (copy the essay) are the most supported. Square pages give sentence starters. Triangle pages are open.

Built For The Student Holding The Page

A "My name" line, one short instruction with a small picture cue, and a colored border. Nothing on a student page is written for an adult: the log, the roll-up, the note home and the teacher prep are on separate pages. Ruled boxes are about a third of an inch a line. The file prints clean in black and white.

Type In, Or Print

The name line, every answer box, every check box and every ruled box are type-in fields in any PDF reader: click one, type, then save. The same goes for the log, the roll-up and the note home. This is a fillable PDF; it is not a Google Slides or PowerPoint file.

Who This Is For

Teachers of grades 4 to 8, special education and resource rooms, counselors, co-taught classes, and substitutes who need a reflection page that is ready to hand out.

What This Is, And What It Is Not

A set of writing pages for a student to reflect on a behavior, with record-keeping around it. It is not a discipline policy, a consequence, a behavior plan, a screener or a clinical tool, and it does not state any school rule: there is a blank line on the Teacher Prep page for yours. Every student and every sentence in the file is invented, and the filled example says so.
