What This Is, And What It Is Not

It is a fillable PDF. Open it in a PDF reader, click a box or line, type, Tab to the next, and Save As once per month. Type the schedule once and the file runs on tally marks and one-letter codes. It works offline with no account or subscription. It is NOT a Google Sheets file, Google Doc, or Excel workbook.

It is not a medical or clinical document. This is the working paper that records when a change happened, who changed the student, and what state the diaper was in. Your school's or district's diaper-change protocol and skin-care policy go on page 3 (the blank line); every rule and every procedure stay yours. The prompt bank is a quick reference; what you actually say to your student is your choice.

Who This Is For

Special education teachers and instructional assistants in early-childhood and self-contained classrooms tracking diaper changes for non-verbal or younger students, case managers documenting care routines, school nurses coordinating diaper-change schedules with classrooms, and families working alongside the school. Anyone who needs to keep a clear record of when changes happened and what patterns are showing.
