About this resource

An IEP goal tracker built around one running page per goal. Write the goal in your own words, add the baseline and target, then record each check as a dated data point and make a dot on the trend graph. Every goal page keeps the whole story of that goal in one place: the numbers, the graph, your trial-by-trial notes, and the date of the next review. A caseload page shows every student and every goal at a glance. Everything is blank for you and your team: this file prints no goal, no criterion and no timeline.

WHAT IS INCLUDED - 18 pages: a caseload page, six goal pages, a review page, a family update, a student page, a phrase bank, a filled example, and a binder cover with tabs

What This Is, And What It Is Not

It is a fillable PDF: open it in a PDF reader, click a line or a cell, type, and Save As once per student. It also prints cleanly in black and white. It works offline with no account or subscription. It is not a Google Sheets file, a Google Doc, or an Excel workbook, and it does not total or graph anything for you: you write the numbers and make the dots.

It is NOT a goal bank and does not write goals. It prints no IEP goal, criterion, mastery rule, reporting rule or timeline: your IEP team and your district write those in. It does not score, compute or label progress. Example students are invented.

Who This Is For

Special education case managers, resource and self-contained teachers, related service providers, and the general education teachers and paraprofessionals who record data between checks.
