About this resource
Keep every NWEA MAP testing window in one place: fall, winter, and spring overall scores and percentiles, a score for each area, the change between windows, growth goals, and a graph the student fills in. Every score, percentile, and area name is blank for you to type from your own report. There is no NWEA content, item, scale, or band table anywhere in this file.
WHAT IS INCLUDED - 18 pages, a setup page, three record pages, a growth goal page, two student pages, a caseload overview, a meeting snapshot, a family update, a phrase bank, a filled example, and a binder cover with divider tabs
Setup
- How This NWEA Tracker Works - four steps and which page to use when
- How the Type-In Fields Work - one page, so nobody emails you, plus what this resource is and is not alongside NWEA MAP
- Setup - 11 settings you write in once (program name, subjects, the names your report uses, who enters scores, how your team sets a goal) and a window planner with 3 rows (fall, winter, spring)
Record Each Window
- Overall Score and Percentile Record - Reading and Math blocks, 4 rows each (fall, winter, spring, retake): date taken, overall score, percentile as your report shows it, notes
- Area Scores by Window - 14 area rows (7 for Reading, 7 for Math): you type the area name, then a score and a percentile for each of the three windows
- Growth Between Windows - 13 rows: fall, winter, and spring scores, then the change from fall to winter, winter to spring, and fall to spring (later score minus earlier score, written by hand)
- Growth Goals - 6 goal rows (subject, starting score, goal score, window, who set it, met or not yet), plus blanks for how your team set the goal and the steps and supports
For The Student
- My Score Graph - a big, friendly graph page the student colors in, with 10 rows for fall, winter, and spring, and a place for the student's own scores
- My Data Chat - a short page in kid words: my scores went up, stayed the same, or down; one thing I am proud of; my next goal; how I will practice; how I feel
Roll Up And Report
- Caseload Overview - 22 rows, one per student, Reading and Math scores for all three windows (use it as your roster, too)
- Meeting Snapshot - one page for a team or family meeting: 4 overall rows and 6 area rows, with space for what the numbers show and questions for the team
- Family Update - a note home in your own words, with a scores table and blanks for what we are doing next and how to help at home
- Observation and Notes Phrase Bank - 17 observable-language sentence starters in 5 groups, each ending in a blank for what you actually saw
- NWEA Tracker, Filled In - an invented fourth grader with 6 overall scores, 5 growth rows and 2 goals, every change checked against its own rows
- Binder Cover and Divider Tabs - a cover and 4 divider tabs (Setup, Scores, Student, Report) to print, cut, and drop into a three-ring binder
What This Is, And What It Is Not
It is a fillable PDF: open it in a PDF reader, click a line or a cell, type, and Save As once per student. It works offline with no account or subscription. It is not a Google Sheets file, a Google Doc, or an Excel workbook. The fields do not link to each other or add anything up, and it does not connect to your NWEA account. The only arithmetic is the change between windows, which you write by hand: the later score minus the earlier score.
It is NOT the NWEA MAP platform, and it does not reproduce NWEA's content, items, score scales, band tables, or scoring rules. It sets no goal and makes no claim about what any score means.
This resource is for use alongside NWEA MAP. It is not affiliated with or endorsed by NWEA. "NWEA" and "MAP" are trademarks of NWEA, used here only to say what this file works alongside.
Example students are invented and say so on the page. Completed pages contain confidential student data, so store them the way your school requires.
Who This Is For
Special education teachers, general education teachers, case managers, and interventionists who keep their own record of NWEA MAP results, and the students who graph their own scores with them.
