About this resource

Track potty training attempts at home and at school on one log: who started it (cued, routine time, or child-initiated), how it went, and progress toward independence. A daily schedule, a daily log with attempts from both settings, accident and success tallies, weekly and monthly summaries, a twelve-week progress graph, an independence tracker that shows the shift from adult-cued to child-initiated, a home-school communication page, and a prompt bank for common situations. Fill in the routine once, Save As per month, then one letter and a tally mark per attempt.

What This Is, And What It Is Not

It is a fillable PDF. Open it in a PDF reader, click a box or line, type, Tab to the next, and Save As once a month. Type the routine once and the file runs on tally marks and one-letter codes. It works offline with no account or subscription. It is NOT a Google Sheets file, Google Doc, or Excel workbook.

It is not a potty-training method or behavioral procedure. This is the working paper that records what happened. Your family's approach and the school's routine go on page 3 (the blank lines); every rule and every strategy stay yours. The phrase bank is a quick reference; what you actually say to your child is your choice.

Who This Is For

Parents and teachers tracking potty-training progress together, early childhood educators in classrooms or programs, families beginning potty training at home who want to coordinate with school, anyone who needs to show weekly and monthly progress toward independence and consistency, and families who want to celebrate progress with a visual graph.
