About this resource

A small group data tracker for the teacher who runs reading or math groups and wants the groups, the sessions and the scores in one place. List your groups and the skill each one works on, add your students, then log every session the way the day allows: a score grid when you have time, a plus/minus trial sheet when you want every try, a one-line quick log when you have two minutes. A group graph shows the shape of the scores, and a regroup page holds each student's decision. Everything is blank for you and your team: this file prints no skill sequence, no passing score and no rule for moving a student.

WHAT IS INCLUDED - 18 pages: groups and roster, three session logs, a group graph, a sheet for another adult, a regroup page, a family update, a student page, a phrase bank, a filled example, and a binder cover with tabs

What This Is, And What It Is Not

It is a fillable PDF: open it in a PDF reader, click a line or a cell, type, and Save As once per group. It also prints cleanly in black and white. It works offline with no account or subscription. It is not a Google Sheets file, a Google Doc, or an Excel workbook, and it does not total or graph anything for you: you write the numbers and make the dots.

It is NOT a curriculum and does not choose skills for a group. It prints no skill sequence, passing score, rule for moving a student, or timeline: you and your team write those in, and the regroup page leaves a blank for them. It does not score or label anyone. Example students are invented and say so on the page.

Who This Is For

Special education teachers, interventionists and resource teachers who run reading or math small groups, and the paraprofessionals and co-teachers who lead a group and hand the numbers back.
