About this resource

A UFLI test tracker you fill in yourself: a blank class and small-group grid for the lesson checks you already give with UFLI Foundations. Roster down the side, lesson checks across the top, retest dates beside the first test, a planner for moving students between groups, and a list of which lesson checks to teach again. This is a recording form, not a test: there is no UFLI lesson name, word list, sentence or script anywhere in this file, and it does not test, score or grade anything.

WHAT IS INCLUDED - 18 pages: setup, three record sheets, two plan pages, a graph, two update pages, a phrase bank, a filled example, and a binder cover with tabs

What This Is, And What It Is Not

It is a fillable PDF: open it in a PDF reader, click a cell, type, and Save As once per group. It also prints cleanly in black and white. It works offline with no account or subscription. It is not a Google Sheets file, a Google Doc, or an Excel workbook, and it does not connect to any UFLI system.

It is NOT UFLI Foundations and does not reproduce any UFLI lesson, word list, sentence or script. It does not test, score or grade anything or set a pass line: every lesson, mark and rule is blank for you and your team. Example students are invented.

Not affiliated with or endorsed by UFLI. This resource is for use alongside UFLI Foundations to record your own lesson-check and retest results. It is not affiliated with or endorsed by the University of Florida Literacy Institute. "UFLI" and "UFLI Foundations" are associated with the University of Florida Literacy Institute.

Who This Is For

Special education teachers, reading interventionists, case managers, general education teachers using UFLI Foundations, and the paraprofessionals who help record checks.
