About this resource
You want the whole week on one page: the periods down the side, Monday to Friday across, a goal the student can say out loud, and a way to tell the family. This is a fillable PDF with four week charts for the teacher, three pages the student fills in, and the pages for the class and the family.
EVERYTHING INSIDE — 21 pages
- Start Here, a contents page with page thumbnails and a "which page for which job" chooser
- Our Goal This Week: room for 3 goals in your words (the goal, what it looks like, what it sounds like), the three marks, a reward line and a blank for your school's own rule
- 4 week charts for the teacher: Week by Period (7 periods, Monday to Friday, 35 mark boxes); Week by Goal (a goal row for each day, then 4 goals marked every day, with teacher and student initials); a Half-Day Week (Morning, Afternoon and Whole day, plus a "what I saw" row); and Week With Notes (a mark, what you saw and a next step for each of 5 days)
- 3 student pages: My Week (a big My name line and 20 stars to color), My Week Check (a face to circle for each of 5 days, plus two writing boxes) and My Goals This Week (3 goals the student writes, 15 boxes to color, and writing boxes)
- A class week roll-up: 17 student rows, Monday to Friday and a week total
- A 36-week log on two pages (weeks 1-18 and 19-36): a line a week for Y, S and N marks, the goal and a note
- A note home with blanks in your own words and a week-at-a-glance table
- A family reply page with room to write back and signature lines for a parent or guardian and the teacher
- A phrase bank of 32 starters in 8 groups
- A filled example: one invented student, one week, and a five-student invented class, every number re-added from its own row
- A binder cover, a spine label and 8 divider tabs (Our Goal, Week Charts, Student Pages, Class Roll-Up, Week Log, Family Notes, Words, Example), edge to edge so the tabs cut apart in two straight cuts across and three down
The marks are plain: Y for yes, S for some of the time, N for not yet. The same three faces are on the student pages, so the teacher's mark and the student's check line up.
What This Is, And What It Is Not
It is a fillable PDF: type into the charts, roll-up, log and notes, or print and use a pen. It works offline with no account or subscription. It is NOT a Google Slides, Google Docs or Google Sheets file, and you cannot drag the boxes around or restyle the pages. It does not add up marks for you; there is a place on each page to write the totals.
It is working paper for counting what you see across a week. It is not a behavior plan, a grade, a point economy or a label for a child. The goal wording, the reward line and what your school expects are blanks for you to write. The student, the class and the teacher in the example are invented and say so on the page. No health information is filled in. Only the binder tabs (page 20) are cut.
